The hard cover book includes a bibliography and an Index, evidently missed in the book binding and carefully inserted in a special envelope attached to the end flyleaf.

The book is in excellent condition, text and dust jacket--just a suggestion of foxing.  The book measures 5 inches by 7 1/4 inches by 3/4 inch and has 271 pages.
